尚未进行特定的临床研究来评估从另一种生物制剂转换为依奇珠单抗治疗强直性脊柱炎/放射学阳性中轴型脊柱关节炎(AS/r-axSpA)。
两项依奇珠单抗研究一项研究的洗脱期是在随机化前(COAST-W),另一项的洗脱期在最后一次阿达木单抗给药后和依奇珠单抗前(COAST-V)。
依奇珠单抗在肿瘤坏死因子抑制剂经治的 AS/r-axSpA 患者的研究 (COAST-W)
COAST-W( N=316)是一项针对于对 1 或 2 种肿瘤坏死因子抑制剂(TNFi)应答不充分或不耐受的活动性强直性脊柱炎/放射学阳性中轴型脊柱关节炎(AS/r-axSpA)患者的为期 16 周、双盲、安慰剂对照 3 期研究,剂量双盲扩展期达 52 周。1

COAST-W 研究的简要安全性结果
在 COAST-W 中,依奇珠单抗 80 mg 每 2 周一次和依奇珠单抗 80 mg 每 4 周一次给药方案的治疗期间出现的不良事件 (TEAE) 的严重程度均为轻度至中度。总 TEAE 和严重不良事件的报告频率与使用依奇珠单抗治疗银屑病和银屑病关节炎患者中报告的频率一致。1
依奇珠单抗在 bDMARD 初治的 AS/r-axSpA 患者 (COAST-V)

在 COAST-V 中最初被随机分配到依奇珠单抗的 164 名患者中,146 名患者(89%)完成 52 周。根据 ASAS 评估,接受依奇珠单抗治疗的患者改善维持至 52 周 。对于最初分配给安慰剂或阿达木单抗并在第 16 周换用依奇珠单抗的患者,ASAS40 应答率增加。对于最初分配给依奇珠单抗治疗的患者,至第 52 周时 ASAS40 应答得以维持()。6
